Students (not parents!) new to high school athletics have created a PlanetHS account.

Physical form completed and signed by doctor.

DCSD registration (physical form submission required).

Registration payment made on MySchoolBucks. *

Booster Club fee paid. *

* Financial aid is available to those who need it.

PHYSICALS

Care Now Urgent Care is offering low cost physicals for $20. Please remember:

• Players should bring all necessary forms with them.

• All students must be accompanied by a parent or legal guardian if they are under the age of eighteen.

• Glasses or contacts must be brought in for sports physical.
